Pressure injuries (also referred to as pressure ulcers, pressure wounds, bed sores or decubiti) can be associated with significant health complications and even death. ODP is working in conjunction with the state’s Health Care Quality Units (HCQUs) to implement a Skin Integrity Initiative. The goals of this pressure injury initiative are twofold: to raise awareness about pressure injury occurrences and to identify appropriate preventive measures that will reduce associated health risks and death. Details are provided below on the implementation, data collection and reporting methods that will be used going forward in this initiative.

Purpose Please see the attached Pennsylvania Department of Health, Health Alert Network (HAN) titled, Public Health Recommendations – Testing, Isolation, and Quarantine by Vaccination Status (HAN-583); which was published on July 30, 2021. This DOH communication provides updates on the following: • This guidance replaces PA-HAN-566 and provides clarification on quarantine recommendations for persons exposed to SARS-CoV-2. • In counties with substantial or high transmission, CDC and DOH recommend all persons, regardless of vaccination status, to wear a mask in public indoor settings. • Fully vaccinated people who have had a known exposure to someone with suspected or confirmed COVID-19 to be tested 2-5 days after exposure, and should wear a mask in public indoor settings for 14 days or until they receive a negative test. • Regardless of vaccination status, any person with new or unexplained symptoms of COVID-19 still needs to isolate and be evaluated for SARS-CoV-2 testing. • If you have questions about this guidance, please call your local health department or 1-877-PA-HEALTH (1-877-724-3258).

Purpose This communication serves to: • Sustain the expectation that AEs, SCOs and providers of waiver residential services use the guidance provided in ODP Announcement 19-091 and the Annotated Individual Support Plan (ISP), dated 7/3/19, to document the staffing support that is needed and will be provided to replace residential habilitation service staffing ratios. • Announce that additional training will be provided in the future to: o Standardize how risk factors and identified health and safety needs should be addressed using person-centered thinking skills, o Clarify documentation of supports and services in the ISP, and o Ensure there is a description of strategies (ex. technology, environmental, and staff supports) to mitigate risk(s) during specific activities and situations.
