Description: This one hour training is specific to Supports Coordinators developing plans for participants. A review of the process in identifying critical areas of need from SIBR results and translating these needs to AAW services will be shared. Additionally, a crosswalk between the SIBR subscales and the ISP module will also be shared in an effort to guide SCs to develop assessment driven, needs based plans.
