Individual Support Plan – promotes an understanding of the value, purpose and requirements for working with teams to develop a person-centered plan.  Outcome Development is a series of courses that begins with a foundational understanding of person-centered outcome statements.  A step by step approach to the completion of the Outcome Section of the ISP is described with numerous examples.  ISP Development and ISP Monitoring include information to help increase understanding of the personal preferences section of the ISP and provide detailed examples and clarification on the SC role in monitoring the ISP.

Dr. Salzer will briefly offer evidence underlying the importance of community inclusion and participation for the health of adults with intellectual and developmental disabilities. He will then introduce the fundamentals for promoting community inclusion, including the knowledge, beliefs, and attitudes required for making it a reality, ensuring the use of conceptually-grounded, evidence-based practices, including development of natural supports, and identifying and addressing environmental barriers to inclusion. He will then be available for questions about how to make these fundamentals a reality in Pennsylvania.

This training is intended to support you in your work with individuals receiving community‐based services and supports and fulfill your role in the County ID Program or Administrative Entity.

Access is limited to registered users with a MyODP Profile role of Administrative Entity (AE) Staff, Administrative Entity Fiscal Staff, Administrative Entity ID Director, or Administrative Entity/ County Administrator.
