Communication and Meetings Structure
AE QM meetings are held at the Greene County Human Services office building on a quarterly basis where activities around CoP are discussed.
Goals, next steps or actions plans
Being a small, single county, but having close ties with larger adjacent counties, I would like for Greene County to be able to participate in a collaborative that would benefit individuals/families that utilize supports, services & resources across county lines. We would also like to host a training in Greene by the PA Family Network for individuals/families, providers, SCO, and other stakeholders to receive education and information about the LC tool and principles.
AE/SCO/Provider/DHS/Broad Community Partners Represented within Collaborative:
Greene AE and SCO have both been trained on the LIfeCourse tool and principles and have incorporated work in this area into each entity's QM plan.
